Just a though, but it may be worth double checking that the barrel is seated correctly, and that the set screw is not self centering and aligning the barrel correctly.

Try this first. It looks like your barrel is not seated in far enough. The extension should extend into the open part of the reciever 1/16 to 1/8 in my experience. If your barrel screw is not centered in the extension dimple, your barrel may be slightly rotated.
Can you wiggle the bolt when it's fully installed? IE take the receiver off the lower. Check the lug on top of the bolt. Have you tried your 223 bolt with the x39 barrel? It should still go into battery.
How about a pic of just the bolt and carrier from the top? If your 223 bolt is working with the 7.62x39 barrel, we'll assume the barrel/extension is not the problem for now.
What we need to look at is the way the bolt/cam are engaging.
That lug is probably the issue. If the carrier has any wear that may make it worse. I'd try RA and see if they'll hook you up. Not sure what they'll chalk up to normal wear. The may have been a bad batch aging the line but it does look like that bolt has seen some Rds
